1. This is an annually recurring sweepstakes promotion, with two (2) prizes being awarded in each sweepstakes year, one (1) prize per six (6) month period (each six (6) month period being a “prize period”). The prize periods expire on July 1 and December 31 of each year. A sweepstakes year begins on January 1 and ends on December 31 of each year. 
2. UWD or its designee will award one (1) prize at the end of each prize period. Each eligible prize winner will receive up to a maximum of $10,000 allowance in UWD exterior home remodeling products (based upon approximate retail value, installed), of which at least fifty percent (50%) shall be used on replacement windows. Unused prize allowance may not be redeemed for cash, and any unused prize allowance will be forfeited. Each prize winner will be determined from a random drawing of all entries received during a sweepstakes, excluding prior winners. Drawings will be held within five business days of the end of each prize period. The odds of winning any prize are determined by the number of entries received prior to the time of the prize drawing.
3. No purchase is necessary to participate in this promotion, and participants do not need to submit to a sales presentation. Void where prohibited by law, rule or regulation. All federal, state, and local laws and regulations apply. UWD reserves the right at its sole discretion to suspend, modify, or terminate this promotion without notice at any time, but any prizes awarded prior to such termination will be honored. UWD reserves the right to amend these Terms and Conditions at any time in its sole discretion.
4. Eligible participants may enter this promotion by one of the following two methods:
   (i) completing the entry form online or by mailing it back to UWD at 7989 Oswego Rd, Liverpool, NY 13090 Attn: Mackenzie Charron; or (ii) printing their full name and complete address on a 3"x5" card, write the words "Universal Windows Direct Exterior Home Makeover Sweepstakes" at the top, and mailing the card to UWD at 7989 Oswego Rd, Liverpool, NY 13090 Attn: Mackenzie Charron. Mailed entries must be postmarked no later than 72 hours prior to the expiration of a prize period to be eligible for that prize drawing. No more than one (1) entry per household is allowed. UWD is not responsible for any illegible, misplaced, damaged or late entries, or technical difficulties. The address listed on each entry form must be the address where the prize is to be installed if such entry is selected as a winner.
5. To be eligible for this promotion, Sweepstakes entrants must (i) be legal residents of New York & be within 1 hour of Syracuse, NY (ii) be eighteen (18) years of age or older, (iii) own their own single-family home, which home has a structure that is in suitable condition (as determined by UWD or its designees in their sole discretion) for installation of the prize (including compliance with local building codes and regulations); and (iv) reside in a geographic area in which UWD operates. Employees and agents of UWD, its affiliates, subsidiaries, co-branded partners, consultants, contractors, advertising, public relations, promotion, fulfillment and marketing agencies, and their immediate families (and those living in their same households) are not eligible to participate in the promotion.
6. Each prize winner will receive the winner’s choice of up to Ten Thousand Dollars ($10,000) approximate retail value of UWD products, of which at least fifty percent (50%) must be utilized for replacement windows, professionally installed by UWD or its designees. Any costs in excess of approximate retail value of the prizes shall be the sole and exclusive responsibility of the winner. Prizes are not transferable by the winner and must be installed at the address listed on the winning entry form. Installation must be provided by UWD personnel or UWD's designee. Prize subject to availability and may be subject to black-out dates. Installation subject to the terms and conditions of UWD's standard installation guidelines. All expenses not specifically provided for are the sole responsibility of the winners.
7. Winner's home must be in suitable condition for products installation (e.g., no water, mold, structural damage, etc.). Any waterproofing, remediation or other repairs necessary for proper installation (as deemed by UWD or its designees in their sole discretion) must be performed at owner's expense prior to installation of the products. Installation labor does not include painting, staining, floor coverings, lighting or HVAC systems. No transfers, substitution in prizes or cash will be allowed except by UWD, who may substitute a prize of equal or greater value in its sole or absolute discretion. Prizes awarded have no cash value. Upon installation of prize, UWD will be deemed to have awarded the prize to the winner with the winner assuming full ownership and responsibility for the prize. Winner must arrange for prize installation no later than one month following the award of the prize and installation must occur and be completed within six months of confirmed notification.
8. The winner must comply with all instructions in the prize notification correspondence, which may include the completion of an affidavit of eligibility and liability/publicity release, as well as have met all of the eligibility requirements set forth within these Rules; otherwise prize will be forfeited and an alternate winner will be selected. Acceptance or use of a prize constitutes permission for UWD and its affiliates to use recipient's full name and likeness for advertising and promotional purposes without compensation, unless prohibited by law. By accepting this prize, the winner agrees that UWD and its affiliates may use the winner's full name, take photographs and video of the winner and his/her family, before and after photographs of the home, and photographs of the remodeling project in progress. Acceptance of the prize constitutes agreement by the winner that UWD may also interview the winner, and the winner agrees to share stories/posts regarding their experience on the winner’s social networking platforms at UWD’s request and subject to UWD’s approval. The winner understands that UWD intends to use their full name, the photographs, video and interviews for the purpose of internal and external promotional materials, marketing and advertising initiatives. Such photographs, videos, and interviews will not be sold to other parties. Promotional materials bearing these videos, photographs or interviews may be posted on UWD or other websites and may be used in any advertising medium, including the Internet, radio and television, and shared on the winners’ social media profiles at UWD’s request and subject to UWD’s approval. UWD reserves the right to use any video, photographer interview for a time period beginning when acceptance of the prize occurs and acceptance of the prize provides UWD with unlimited use of these items.
9. Liability for any applicable taxes, including federal, state, and local taxes and/or other fees that may apply to prizes are the sole responsibility of the winner. Any required tax reporting forms will be furnished to the Internal Revenue Service. All expenses not specifically provided for by UWD in these Rules are the sole responsibility of the winner. Except where prohibited by law, as a condition of participating in this promotion, each participant agrees that: (1) any and all disputes and causes of action arising out of or connected with this promotion shall be resolved individually, without resort to any form of class action, and exclusively by final and binding arbitration under the rules of the American Arbitration Association and held in Cuyahoga County, Ohio; (2) the Federal Arbitration Act shall govern the interpretation, enforcement and all proceedings at such arbitration; and (3) judgment upon such arbitration award may be entered in any court having jurisdiction. Under no circumstances will any participant be permitted to obtain awards for, and participant hereby waives all rights to claim, punitive, incidental or consequential damages, or any other damages, including attorneys’ fees, other than customer’s actual out-of-pocket, and customer further waives all rights to have damages multiplied or increased.
10. Entries that are lost, late, illegible or incomplete for any reason will not be eligible. UWD reserves the right to modify or terminate this Sweepstakes at any time or disqualify any entry in good faith. In the event of a dispute concerning the identity of the person submitting an entry, the entry will be deemed to be submitted by the person in whose name the winning home is titled.
11. UWD reserves the right, at its sole discretion, to disqualify any individual it finds to be tampering with the operation of the promotion, to be acting in violation of these Terms and Conditions, or to be acting in disruptive manner, or with the intent to annoy, abuse, threaten or harass any other person. Any person attempting to defraud or in any way tamper with this promotion will be ineligible and may be prosecuted to the full extent of the law. 
12. By participating in the promotion, participants agree to release, discharge and hold harmless UWD, its affiliates, co-branded partners, subsidiaries, retailers, agents and their respective officers, directors, employees and agents, from any damages which may arise out of participation in the promotion in any way or out of acceptance, use, misuse or possession of a prize. Acceptance of a prize may require entering a standard contract or purchase order for the goods and services to be provided, as required by law. This promotion is conducted in English (which will be given its everyday ordinary meaning). This Sweepstakes is sponsored by Universal Windows Direct of Syracuse, Inc. 7989 Oswego Rd, Liverpool, NY 13090 and governed by the laws of the State of New York with exclusive venue/jurisdiction in Onondaga County, NY.
